To whom it may concern,

My mother has been the recipient of care from your company, At Home Health, for the last two months. I wanted to take this opportunity to express my gratitude and appreciation for the care she has received from your staff.

During her time with you, she has received PT, OT, HHA and nursing care (Carol, Kathleen, Justina and Amanda were her main care providers). There were times when your staff went beyond and above the call of duty by providing support and friendly advice when mom became ill with walking pneumonia and experienced a three day hospitalization. Carol was the last person who provided care with mom as she was becoming ill. Carol not only helped me discern mom's status but provided helpful advice for my decision making when taking mom to the emergency room. I greatly appreciate having someone to process thing with. Later, she called and checked in with me about mom's status multiple times during her hospital stay. Your HHA, Justina, Provided relaxed, careful care which was simply outstanding.

All of the staff not only cared for mom, but carefully provided training tips for me as I resumed more of her home care. I now feel empowered to care for mom on my own. They all showed competence in their skill and I learned a lot from them.

Every time the staff were in my home, they were king, friendly, patient, and simply a pleasure to be around. I never felt that they were rushed and they always stayed until all of mom's care was complete and all of my questions were addressed. I will miss seeing their faces.

Given the need, I will certainly contact At Home Health Services for future service, and would not hesitate for a second to recommend you to a friend or family member.

Programs

At Home Health Services strives to be on the cutting edge of programs our patients require and deserve for their continued care. We currently offer the following programs:

Neuro-Feedback: Brain Wave Mapping

At Home Health Services is aggressively working with four behavior psychologists and two neurologists to continue identifying ways to develop programs that assist with memory care for those patients and their caregivers. Through a process called Neuro-Feedback, this process works through a learning process called operant conditioning in which reward feedback (auditory/visual) informs the patient when their brain waves are acting in a healthier way that is improving cognitive and behavioral functioning. Through therapy, the patient is taught the cognitive skills and strategies necessary to manage their symptoms. This combination of Neuro-Feedback and cognitive/behavioral therapy will teach patients how to use their mind to change their brain wave state. This empowering process is known as self-regulation.

EMR: Electronic Medical Records

At Home Health Services is staying on the forefront of technology by implementing electronic medical records. Electronic medical records, allows all clinician and administrative paperwork and physician orders to be managed with a click of a button. ATHHS is transforming the way documentation is managed: from beginning to end, all documentation is managed through secure software that allows ALL parties involved to manage all patient records more effectively.

Telehealth:

Telehealth is the delivery of health related services and information via telecommunication technologies. Telehealth allows patients to administer and check their vitals in the comfort of their homes. Whether you have; CHF, COPD, Diabetes, Hypertension, Asthma, CKD and ESRD, Telehealth allows your health care provider and At Home Health Services to actively monitor and make the necessary changes to your condition.

Interpreter/Translator Services - 150 Languages:

At Home Health Services is committed to making sure that we service you or your loved one in the most efficient manner possible. By providing excellent care while in the comfort of your home, we are committed to offering services such as interpreters and translators for over 150 languages. At Home Health Services takes the guess work out of how we can provide our services to those patients that speak a language other than English. By providing this service, we are able to capture the most appropriate plan of care, so that you or your loved one is given the same services without letting their language be a barrier from the care they deserve.

Complex Disease Management:

Whether you have diabetes, COPD, CHF, Paralysis or need Infusion Therapy, At Home Health Services is committed to treating these conditions while in the comfort of your home with our committed and well trained staff (maintaining their continuing education credits according to state and federal requirements). Our clinicians average over fifteen years of home-care experience in evaluating, treating and helping patients with many of these and other conditions. At Home Health Services ensures that all conditions are treated according to medicare and medicaid services and many national advisory medical societies.

Fall Prevention:

Each year, thousands of older Americans fall at home. Many of them are seriously injured, and some are disabled. Falls are common in 1 in 3 senior falls each year. Falls increase with age-as many as 40% of seniors over 80 experience a fall. Falls are often due to hazards that are easy to overlook but easy to fix. That is why At Home Health Services offers a Fall Prevention program call; "Building Better Balance-B3" that assists with these hazards in your home to ensure that you or your loved one is in a very safe and livable environment. Let At Home Health Services assess your home for these potential hazards and offer recommendations and treatment interventions to recover balance strategies, and problem-solving skills in order to ensure the highest level of independence is achieved for your loved one.
